1. Acknowledgment of Risks

I, _______________________________________, understand that participating in Brazilian Jiu-Jitsu (BJJ), martial arts, and related physical activities at Eastland BJJ involves inherent risks, including but not limited to:

•  Physical injury, such as bruises, sprains, strains, fractures, dislocations, or concussions.

•  Serious injury, including permanent disability, paralysis, or death.

•  Risks associated with physical exertion, contact with other participants, or use of equipment.

•  Risks from the gym environment, including mats, training areas, or other facilities.

I acknowledge that these risks may result from my own actions, the actions of others, or the condition of the premises or equipment.

2. Assumption of Risk

I voluntarily choose to participate in activities at Eastland BJJ and expressly assume all risks associated with these activities, whether known or unknown, foreseeable or unforeseeable.

3. Release and Waiver of Liability

In consideration of being permitted to participate in activities at Eastland BJJ, I, for myself, my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and agree not to sue Eastland BJJ, its owners, officers, instructors, employees, agents, or affiliates (collectively, the “Released Parties”) from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or to any property belonging to me, whether caused by the negligence of the Released Parties or otherwise, while participating in activities at Eastland BJJ or on its premises.

4. Indemnification

I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or costs, including attorney fees, that they may incur due to my participation in activities at Eastland BJJ, whether caused by my negligence or otherwise.

5. Health and Physical Condition

I represent that I am in good physical health and have no medical conditions that would prevent me from safely participating in jiu-jitsu or related activities. I agree to inform Eastland BJJ of any medical conditions, injuries, or limitations that may affect my participation. I understand that Eastland BJJ is not responsible for determining my medical fitness to participate.

6. Emergency Medical Treatment

I consent to receive emergency medical treatment in the event of injury, accident, or illness during my participation at Eastland BJJ. I agree that Eastland BJJ is not responsible for any costs associated with such treatment.

7. Rules and Conduct

I agree to follow all rules, instructions, and safety guidelines provided by Eastland BJJ and its instructors. I understand that failure to comply may result in my removal from activities without refund.

8. Photo/Video Release

I grant Eastland BJJ permission to use photographs or videos of me taken during activities for promotional or instructional purposes, unless I provide written notice to the contrary.

9. Severability

If any provision of this waiver is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

Acknowledgment of Understanding

I have read this waiver and release of liability, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing this agreement freely and voluntarily, and intend it to be a complete and unconditional release of all liability to the greatest extent allowed by law.
